What has gone wrong with application development? Who is the culprit?

Large applications have been developed with paradigms, methods and guidelines which have proved useful for small ones. That is one of the reasons of many deficiencies. The major culprit may be our orientation on gettingprofits as soon as possible. This makes it necessary to think and act on a longer term basis.
